- Your referral letter from your GP or specialist
- Your Medicare card and private health insurance details (if applicable)
- Any recent test results, scans, or reports related to your condition
- A list of current medications you’re taking
- Your completed patient registration form (sent to you before your appointment)
It’s also helpful to write down any questions you have so you don’t forget to ask them during your consultation.
